"All those who have at any time given clinical demonstrations on disease must have felt the desire to impress more firmly on their hearers the remembrance of what they have seen than is possible in an ordinary lecture. After many attempts to arrive at this in some other way, I have now tried to preserve, in a measure, the impressions of a term’s clinical work in the form of the lectures contained in the following pages. In my descriptions I have endeavoured, as far as possible, to follow the actual course of the lectures. Naturally, not only must this work give up all claim to the actual presentation of the patients, for which the student can only be compensated by personal experience in the hospital, but we must also forego the great help to teaching afforded by the assistant’s little awkwardnesses and blunders, which so often serve to point out to the teacher the right method of instruction. On the other hand, the material can in this way be worked up more concisely, more systematically, and more completely than is generally possible in the hospital.In these lectures I have always kept the diagnostic point of view in the foreground, being convinced of its fundamental importance, not only to our scientific ideas, but also as affecting the advice we shall have to give in our medical practice, and the methods of treatment to be adopted by us. In my opinion, what the student ought to learn in the hospital, besides the examination of patients, is not text-book knowledge, which he can acquire just as well, or better, at home, but how to turn his observations to account, and the careful judgment of any given case. These lectures, then, must not in any way be looked upon as a textbook of alienism. Their aim will be far better attained if they prove of some value as a guide to the clinical investigation of the insane.The examples of disease, which for obvious reasons have been taken from entirely separate years, and some of which have been utilized elsewhere, do not profess in their brief outline to be scientific records. Nevertheless, each individual case is delineated with the greatest possible truth to life, while the diagnostic developments are almost entirely taken directly from notes upon clinical demonstrations. Esta obra, que intentó sintetizar los desarrollo de la Clínica Psiquiátrica del siglo XIX y marcó indeleblemente la del siglo XX, cumple en este momento cien años de existencia, desde la aparición de las 5ª y 6ª ediciones del texto de Krepelin publicados entre 1895 y 1900. El conocimiento de la versión original - por primera vez traducida al castellano - del texto de La Demencia Precoz constituye una herramienta indispensable para el psiquiatra contemporáneo, a fin de comprender críticamente los presupuestos de las más recientes clasificaciones psiquiátricas y el origen de las nociones implicadas que hay en la discusión clínica sobre la esquizofrenia.

a selection from the beginning of the first chapter: PARANOIA, OR PROGRESSIVE SYSTEMATIZED INSANITY Gentlemen,—Among the symptoms of insanity, delusions and hallucinations, very often associated with each other, have in particular attracted attention, along with the fluctuations of mood. The different forms of delusion—the delusion of greatness and insignificance, the delusion of sin, the physical delusion of persecution, and so on—have often been considered as characteristics of distinct forms of disease. According to our present experience, conclusions as to the clinical meaning of a picture of disease from the existence and purport of the delusions are only permissible to a very limited extent. The tendencies of human fears and wishes can follow the same morbid ways in the most varied forms of insanity. But yet, the nature of the disease remains naturally not quite without influence upon the form of the delusion. In the following lectures we shall have, therefore, somewhat more minutely to observe some relations of this disturbance to definite forms of disease. The stately gentleman, aged sixty-two, who presents himself before us with a certain courtly dignity, with his carefully-tended moustaches, his eye-glasses, and his well-fitting if perhaps somewhat shabby attire, gives quite the impression of a man of the world. He is somewhat testy at first because he has to allow himself to be questioned before the young gentlemen, but soon enters into a long, connected conversation in a quiet and positive manner. We learn from him that as a young man he went to America, and there went through many vicissitudes, finally settling in Quito, where as a merchant he made a small fortune. With this he returned home twenty-one years ago, but on the dissolving of his business connections he was done out of considerable sums. At home he lived at first on his money, spending his time in amusements, reading the newspaper, playing billiards, going for walks, and sitting about in cafés. At the same time he occupied himself with all sorts of schemes from which he hoped for recognition and profit. Thus, he submitted to the leading Minister the plan (with a map) whereby Germany could lay claim to a lot of still unpossessed land, especially in Africa and New Guinea, and, above all, the islands of Galapagos, which the State of Ecuador would willingly hand over, and which would become of great importance after the finishing of the Panama Canal. A short time after that same Minister travelled to Berlin, and now began the German Colonial Policy, without, it must be confessed, due thanks falling to the lot of the real originator, which another nation would not have withheld. Then our patient drew up a plan for the cultivation of cinchona and cacao in our colonies; he also made several inventions for the better connection of railway-metals, by which the jolting, an important cause of derailment, would be done away with. Finally, he applied for a number of situations which seemed suited to him, including that of the consulship at Quito, but had always only failures to record.

When Emil Kraepelin died (15. 02. 1856-07. 10. 1926), he left behind his memoirs, which were not published until today. Co-workers and col leagues from the last years of Kraepelin's life and his family knew that such an autobiography existed, but they were not sure whether these memoirs should be made public, so they remained unpublished. At a commemoration 50 years after Emil Kraepelin's death in Mu nich (1976) the descendants, who attended the festivities, agreed to the editors' suggestion to publish the "Memoirs". Through this publication we have been able to learn more about the life of a man, whose work has had a decisive and lasting effect on the entire psychiatry since the end of the last century. When one reads the memoirs, one has to admit that on ly very little is known about this great psychiatrist's personality and path through life and that this gap has by no means been filled by the publica tion of his memoirs. Perhaps this publication will encourage work on a biography, which will give us more information on Kraepelin's person ality, life and scientific work. The focal-point of Kraepelin's work lies in the classification of psy chiatric clinical syndromes, which was developed from clinical observa tions, was constantly tested empirically and was altered taking new findings and knowledge into account. However, the basis of this system has remained unchanged and valid until today.
O que distingue a obra de Kraepelin dos seus predecessores e contemporâneos foi o rigor metodológico na colheita dos fenómenos clínicos, uma permanente atitude crítica e analítica relativamente às próprias concepções, mas principalmente a sabedoria da sua síntese clínica. Acima de tudo, este terceiro volume traz ao leito português uma obrigatória descrição clínica e conceptualização da doença afectiva que é inigualável na prática da psiquiatria clínica.

Le diverse “entità cliniche” sono divenute “forme”, cioè quadri clinici relativamente tipici ma che non sono sostenuti da processi eziopatogenetici univoci. In questo testo è palese la presa di distanza dal mito positivistico delle unità di malattia in psichiatria. In questo testo Kraepelin anticipa sorprendentemente molti dei modelli psicopatologici e clinici delle psicosi che sono emersi molti decenni dopo.

As lições clínicas de Heidelberga foram editadas num texto de Kraepelin de 1905 designado Introdução à Psiquiatria Clínica. Da totalidade das 32 lições foram escolhidas as 10 mais representativas das patologias que por Kraepelin foram desenvolvidas nos subsequentes tratados de Psiquiatria. Estas «lições» são de uma grande riqueza de observação clínica. São os relatos da apresentação dos doentes aos alunos durante um semestre que eles passaram na clínica de Heidelberga a estudar com Kraepelin. Pode constatar-se nestas lições a capacidade de síntese semiológica de Kraepelin, de captação do essencial, de rigor na descrição. A sua tendência é sempre de procura de um diagnóstico, de uma classificação dos fenómenos observados em formas mórbidas. Com essa finalidade dá a maior importância ao curso das doenças e aos antecedentes clínicos de cada doente. Estas lições complementam de uma forma admirável os textos de Kraepelin que têm sido editados em português, o seu método de análise e o seu raciocínio clínico.

Obgleich jene Arbeit nur den ersten Schritt auf einem langen Wege bedeutete, war ihr Ver tasser dennoch in der Lage, bereits über eine stattliche Anzahl von Ergebnissen berichten zu können, die uns zeigen, nach wie vielen Richtungen hin die weitere Aufschließung des betretenen Gebietes Erfolge verspricht. Fiir den Irrenarzt ist es von besonderer Bedeu tung, dass die Lösung der für die Versuchsperson ungemein einfachen Aufgaben die sehr genaue Zergliederung einer geläufigen Willens handlung gestattet. Willensstörungen spielen auf dem Gebiete der geistigen Erkrankungen eine sehr große Rolle, entziehen sich. Aber zur Zeit unserem wissenschaftlichen Verständnisse noch in weit höherem Grade als etwa die krankhaften Veränderungen der Auf fassung, des Gedächtnisses, des Gedankenganges. Excerpt from Psychologische Arbeiten, 1896Seit einer längeren Reihe von Jahren habe ich mir die Auf gabe gestellt, die Methoden und Ergebnisse der psychologischen Forschung für diejenigen Wissensgebiete nutzbar zu machen, welche meinem Gesichtskreise am nächsten liegen, vor Allem für die Psy chiatrie. Diese Bestrebungen, die allmählich immer festere Ge staltung gewonnen haben, mussten nothwendig zu einer groà en Zahl von Einzeluntersuchungen führen, welche bisher zumeist als Vorarbeiten anzusehen sind, zum Theile auch schon beginnen, die vorgezeichnete Aufgabe selbst anzugreifen. Die erste ist die ganz auà erordentliche Flüchtigkeit der Erinnerung an den Wortlaut der à uà erungen. Auch die Spuren anderer Traumerlebnisse p?egen rasch verloren zu gehen; immerhin aber sind lebhaft träumende Menschen doch ganz gewöhnlich im stande, eine Reihe von Einzelheiten aus ihren Traumereignissen fest zuhalten. Dagegen haftet von den Sprachlichen à uà erungen ohne besonders darauf gerichtete Aufmerksamkeit in der Regel gar nichts, ja, es ist meist trotz der äuà ersten Bemühungen nicht möglich, ihren Wortlaut dem Gedächtnis einzuprägen, wenn man sie nicht sofort nach dem Erwachen schriftlich aufzeichnet. Oft genug ist es mir begegnet, daà ich ein Beispiel, welches ich zufallig nicht nieder schreiben konnte, beim Aufwachen durch ungezahlte Wiederholungen vergeblich auswendig zu lernen suchte. Auch wenn ich es mir ganz sicher eingeprägt zu haben glaubte, muà te ich kurze  zeit darauf zu meinem Verdrusse die Wahrnehmung machen, daà es mir dennoch völlig und unwiederbringlich entschwunden war.
